Introductions with staff from the different departments within the center. Reviewed WIC training materials (Participant-Centered Nutrition Services, Participant-Centered Nutrition Assessment, and others) on WICSIS system. Received Ryan Center policy paperwork at the HR office.

 

Comments / Thoughts / What You Learned / Challenges:

The Ryan Women and Children’s Center, part of the William F. Ryan Community Health Network, is located on the Upper West Side of Manhattan at 801 Amsterdam Avenue. This site contains a Women's Health Department, Pediatric Department, Mental Health Department, Prevention and Outreach Services, and the WIC Program. After reading the Participant-Centered presentations I had a better understanding of the strategies used to provide quality care by all WIC department staff. This is accomplished through building relationships with participants by building rapport and making them feel empowered to make healthy choices. Focus should be placed on a participant’s strengths and capacities to increase motivation to change and the likelihood of success. Other useful strategies include: open-ended questions, active listening, affirmations, and reflective listening. By keeping the participant’s needs in mind, staff is more likely to be successful in assisting participants in desired behavior change.

I am excited about the opportunity to participate in a community outreach event this early in my rotation with the Ryan Center. Marlene and I have decided to use a presentation put together by the outpatient dietitian, Angelica, which covers healthy eating habits. We modified the presentation to be more relevant to our audience. We’ve been informed that the target audience is made up of middle-aged, African-American and Hispanic men. We are focusing the presentation on diabetes, heart disease, and hypertension.

Today’s presentation at the Phoenix House went well despite that fact that the audience contained three women that we were not aware would be in attendance. This was a good lesson and one that affirms what Sandra at SBHC told me about being ready to face any challenges that come your way in community nutrition.

I had the chance to observe Charlotte, a recently hired nutritionist, with two participants. I was surprised at how fast the appointments were conducted. Since neither of the two appointments were for initial certifications they were able to be done quicker. Both participants were asked the standard questions and given the opportunity to express any concerns they had. Neither participant had any concerns which allowed the appointments to be done faster.

One of the tasks I have been given is to develop recipes for participants based on ingredients provided in the food package. A board in the hallway to the nutritionist’s offices contained four recipes that need to be updated. I am looking up ideas for recipes that include foods provided to the participants that are also available now during the Spring season.

The four recipes that I have developed are: Black Bean and Salmon Tostadas, Brown Rice Burgers, Quick Breakfast Tacos, and Creamy Sweet Potato Soup. These recipes all contain at least one ingredient provided in the food package and the Quick Breakfast Tacos are made up entirely of ingredients in the package. The intention of this is to encourage participants to make the recipes at home with ingredients provided by WIC.

I have been working on the SAB for the past two days and I have come across an old idea that I would like to re-establish. I included the local branch of the public library in the SAB since it was not in last year’s version and spoke with Pam about the possibility of our office and the library holding a joint event. Pam told me that they used to hold these events in the office and that she would like to restart them. I think that a partnership between the NYPL and WIC is a beneficial endeavor for both agencies. Pam has put me in charge of organizing the event which I have started to do so by meeting with Jessica at the library.

I continued to work on finding new activities to add to the SAB. I am also removing any old activities or events that are no longer being offered. This booklet has a lot of great information and will be a valuable resource for the participants when looking for free and/or low-cost activities this summer.

I had the opportunity today to answer a participant’s questions on cooking quinoa. She seemed apprehensive to attempt to try cooking a new ingredient that she knew little about. I was able to describe the process to her and assured her that it is something that she will be able to do. She told us that she would try to cook the grain soon and that she felt more confident in her ability to do so.

In addition to the Health Bucks that participants are already eligible for if they spend their WIC Fruit and Vegetable check at farmer’s markets I have informed Pam that additional Health Bucks can be obtained and distributed to the participants. From this morning’s meeting I learned that the DOH has coupons that they will provide to various organizations helping people in need of extra assistance in purchasing fresh produce. I gave Pam Ms. Abrami’s contact information for the WIC office to be able to obtain some of these coupons being offered for our participants.

One of the participants I saw with Orly was in for a check pickup. The other was a HNU and Orly provided her with information on limiting her child’s juice intake due to the child’s overweight status. The participant was informed that juice served could be dilute with water a little bit at a time until the child is drinking one part juice to one part water. The participant accepted this recommendation as something she could do.

Today I had the opportunity to participate in a breastfeeding support group meeting with breastfeeding peer counselor, Regine. Three participants attended the meeting out of five that were scheduled. Regine explained to me that this was a successful showing and that most meetings only get one or two participants to show. The meeting lasted for about an hour and the participants were provided with a lot of information as first-time breastfeeding mothers. The groups serve as an opportunity for participants to discuss common concerns and for the peer counselors to offer support. The participants were all given instructions on calling in with any questions they have and were informed that a peer counselor would be calling weekly to check in with them. By offering as much support as the Ryan Center does to breastfeeding mothers they ensure that they’re following their statement that the initiation of breastfeeding is their priority.

Observed three participants with Orly. Provided a participant with information on introducing solid foods to a newborn. Started formatting 2014 version of SAB.

I had the chance to participate in another group setting today. I helped Charlotte lead a facilitated group discussion with participants that were waiting to pick up their checks. The purpose of these groups is to provide nutrition education in an informal setting with participants that would otherwise be sitting in the waiting room for their appointment. The meeting today was focused on ideas for the participants to try with their children to get them outdoors on a picnic. We asked the participants what types of food they usually bring with them for an outdoor event and we also discussed physical activities that could be done. The three participants in the group all offered their own ideas and Charlotte and I provided encouragement and tried to combine their ideas with some that we came up with ourselves to increase the options the participants could work from. The group was successful in engaging the participants and provided useful information.

The office was short staffed today so Pam asked me if I would help the nutritionist assistants with taking heights and weights. I learned the method for measuring length of infants and height for older children. I also learned the method for recording weight on both the table scale and the floor scale.

Today’s Fit WIC event was a lot larger than I expected it to be. We had about 15 children and their parents attend the event. The reason for such a good turnout was the phone calls Elizabeth made to remind participants that the event was being held today. The snack activity was very successful. Before the last child had finished making their snack and I had a chance to address everyone after helping with assembling most of the children had already eaten their snack! I was expecting to have to provide some encouragement and make the activity fun by suggesting the children eat the bunny’s “ear”, made from bananas, but no encouragement was needed. For the next event I will make sure to ask the children more questions as they’re assembling their snack to give them an opportunity to learn more about what they're doing and eating.

Everyone thought that the Fit WIC event was a success. The idea of Elizabeth’s to make the reminder calls will be continued for future events. Increasing the number of participants that attend these events allows the office to run more efficiently by consolidating check pickups and other appointments. It also enables us to provide a short nutrition lesson to a larger number of participants which is another great benefit.

In an effort to promote National Infant Immunization Week I created a handout for the participants. This message ties directly into processes here in the office, specifically the question the nutritionists ask the participants about their children being fully up-to-date with vaccinations. This is an example of the level of comprehensive care provided by WIC and the effort to care for the person as a whole. I found the process of creating a half-page handout on the importance of immunizations to be challenging but using the space on the front and back of the sheet I was able to condense the most important messages for the participants to receive.

I have been able to find a lot of great ideas for games participants can play in their house using materials they have on hand. Some of the games do not include any material at all while others use items such as water bottles, paper plates, tissues, and other common household items. We are planning on testing some of these games at the next Fit WIC event and I will develop a handout for participants to have for their own use.

I had the opportunity today to create a spreadsheet that will serve as a log to track staff training for the next three years. I utilized employees records to determine training credits already earned by each staff member. I provided this spreadsheet to Pam who will include it in her records to have on file for future state inspections. I was grateful for the opportunity given to me by Pam to develop this document for her use as the director.

I was very surprised to learn today the extent of how little variance there is in the federal poverty guidelines. Besides Alaska and Hawaii, every other part of the U.S. falls under the same guidelines. These guidelines do not take into consideration any other geographical information when this greatly impacts financial needs, especially rural vs. city living. An approach that factored in a person or family’s zip code and actual cost of living for that area would be much more realistic.

I was glad to finally have the new recipe cards on display in the office. The old recipes were no longer being taken so this was a good time to make the switch. I updated the board to make it more appealing to the eye in an effort to increase usage. It will be interesting to monitor the board over the next month to see if some recipes are taken more than the others.

A task that I have yet to accomplish during my time here so far is to develop a new bulletin board in the waiting room. The waiting room has a very large (approximately 4’ x 15’) bulletin board that is directly in front of the chairs the participants use. It is a great way to expose a large number of participants to a healthy message. In conjunction with the upcoming screen-free week event I have decided to create a bulletin board focusing on decreasing or totally eliminating screen-time and increasing physical activities both indoors and out as well as other activities that do not involve a screen.

Today was a joint staff meeting between the two WIC offices in the Ryan Network. All of us from the Upper West Side location went to the other location in the Lower East Side. We were given a tour of the newly relocated WIC office within the LES location and attended the staff meeting in their conference room. A representative from Delta Dental gave a presentation on oral health care which is a message that WIC already provides to participants to prevent dental caries in children. The rest of the staff meeting focused on processes of each location and ways in which they can be standardized between the two offices.

It was challenging to come up with ideas for Infant Fit WIC sessions that would be exciting and educational for the children while also remaining with the budget allotted. I based my decisions on the past events that I have been involved with the activity portion and how successful those events were in remaining on budget. Also, the process of buying the materials for an acitivy and observing the reactions of children in the age group it is targeted to was a useful strategy in the development of other ideas for the groups. I found out that simple ideas that do not cost a lot of money can be fun and educational such as the lava lamp.

Today I saw the completion of two projects that I think will have a large impact on participants of this office. The bulletin board offers information on the benefits of reducing screen-time for children including: weight loss or maintenance of a healthy weight, better performance at school, better behavior at home, among others. Many great alternatives are given to help occupy children’s time while they’re not on a device. The SAB has a strong connection to the bulletin board as it offers a very large variety of activities and event for families to try. It will be handed out to participants now that summertime events have begun in the city.

Based on the snack activities that I’ve led here and at SBHC I have developed a short nutrition lesson to conduct during the food demo. I am going to discuss each ingredient being used, the fact that they are all included in the food package, and the benefits of them as well as the overall benefits of eating breakfast. I want to convey the importance of breakfast to give the participants the motivation to prepare breakfast for themselves and their children if they do not already do so. Even for the participants that do, this recipe will provide a healthy idea that they could incorporate into the others they prepare.

The NYPL event is starting to come together and will be held next Thursday here in the office. I have created a flyer to post in the center as well as at the library to inform people of the event. I have also requested for Elizabeth to call participants to inform them of the event. The plan is to have around the same number of participants at the last Fit WIC present for this event.

I thought that making an example of the snack for the NYPL event would be a good way to increase the appeal of the flyer for the event. I also was able to more accurately assess the amount of food needed for the snack activity. The recipes that have been the most popular so far are the tostadas and the breakfast taco. Not as many participants have taken the brown rice burger or soup recipes.

The participant I saw today with Marlene was the mother of a girl who's BMI is almost twice that of the obese line. The mother gave a diet recall and described her daughter’s physical activity level. Marlene recommended a goal of transitioning to 1% milk and increasing PA. The mother was provided with handouts on physical activity and games. Something that made this case even more interesting was the fact that the girl’s appearance was not an obvious indication of her weight status. She appeared to be much closer to a normal weight than her chart indicated. Marlene and I discussed the possibility of an error made during her height and weight measurements. It would be acceptable to reweigh and measure a child if their chart does not match their physical appearance to ensure accuracy.

In anticipation of farmer’s market checks that we’ll be handing out to the participants next month I have updated and put up posters informing participants of the nearest five farmer’s market to the office. This information along with the various other handouts that will be provided to the participants will hopefully enable them to fully utilize their farmer’s market checks. Jessica at the Bloomingdale library branch was appreciative of the flyers for our event and is ready for the event next week.

I had the opportunity today to test my skills again with occupying the interest of a large group of children when appointments were delayed. I brought over some books to the pediatric department and read with the children. At first I read out loud to them and then I gave each child a chance to read a page. They were excited about showing off their reading abilities and the older children were helpful with the younger children that were not as advanced as them. I also brought over some copies of the recipes from the board in the hallway for the children to give to their parents.

The last of the planning for the NYPL event is done and everything is in place for Thursday. I am concerned that the turnout may not be as high as we’re hoping for since the event is planned for 11AM. There has been more success with turnout by holding events later in the afternoon but the children’s librarian is only available until noon which requires for us to hold the event at the earlier time.

I have completed the Infant Fit WIC handout for Pam to review. There will not be time for an event to scheduled while I am here but she will use the ideas to plan an event once the staff has had a chance to review the ideas and decide on which ones they would like to do. I would have liked to have been a part of these events but it is enough knowing that they will be held and the information I have provided will be passed on for the participant’s knowledge.

Set up waiting area for NYPL event. Prepared snack, helped put gifts bags together for children (pajamas/books). Assisted children in making the snack. Asked them what their favorite fruit/vegetable is, asked them to identify the fruits and vegetables being used to make the snack. Encourage parents to sign up for library cards with Jessica, the librarian. Clean up after the event. Sent thank you emails to Adena and Jessica.

I was able to do a much better job of engaging the children during this event compared to the last Fit WIC. I made sure to talk with each of them and their parent(s) as they prepared their snack so that they receives the nutrition message that went along with the snack activity. The process was made easier also by the fact that the participants that attended the event were staggered and we never had more than four people at a time. We encourage patients in the pediatrics and women’s health departments to join the event and were successful in getting several to participate. Overall, 18 people made a snack and almost as many signed up for library cards. We ended up holding the event until after 1 PM, despite the librarian having to leave, in order to use up the rest of the snack ingredients. We had the librarian leave us information that we were able to hand out to participants throughout the remainder of the event.

I spent the day today working on new recipe cards. These cards will replace the spring cards and will provide new recipes for the participants in the interim period between interns once I finish my rotation and the next intern starts at a later date. The recipes include: Chicken Kebabs with Creamy Pesto, BBQ Tofu Sandwich, Summer Corn and White Bean Soup, and Blueberry Banana Oat Bread. These summer recipes also contain ingredients found in the WIC food package and are seasonally based.

In an effort to help reduce the workload for the nutritionist assistants I translated as much as I could of the recipes cards into Spanish. The office is short-staffed today so I took it upon myself to begin this process and used it as an opportunity to enhance my knowledge of the Spanish language. It is apparent that knowing a second language is very beneficial in the community setting and I am attempting to learn Spanish.

The food demonstration was a success! The number of participants who attended could have been a little higher but the office was a little slow this afternoon due to a few no shows. I made a similar effort to the NYPL event by encouraging patients from other departments in the center to come see the food demo. I ended up talking with 15 participants throughout the demo. I received mostly positive feedback on the recipe. A few participants expressed a dislike of one or more aspects of the recipe and I was able to discuss some alternatives with them that would increase their willingness to try the recipe on their own.

I am working on tasks this week, which is my final week here at the Ryan Center, that will help the staff throughout the next month or so before the next intern arrives. Just as I’ve created new recipe cards to put out I’m also working on a new bulletin board that can be displayed for a while before needing to be replaced. The topic, food allergies, is one that I am very interested in and would like to learn more about. Pam has informed me that they have never had a bulletin board on the subject and that it would be a good topic to work with.

Today Ciriah and I had a very interesting conversation on some common misperceptions of breastfeeding mothers. She mentioned the concerns she hears raised most often and expressed a desire to have a handout that could explain visually what she tells the participants she talks with on the phone each week and in the support groups. I put together a handout for her in an effort to explain that breast milk contains the amount of water an infant needs which is a common concern she hears. The handout also showed that breast milk contains carbohydrates, fats, and protein to provide the baby with the proper amount of nutrients throughout their first year.

I got more Spanish practice in today by looking for translations of key points I have found in English. I was able to find translations for everything and the completed bulletin board is on display. I feel a sense of pride in completing this bulletin board, as I did for the previous one, because of the number of people who will see and read it over the time that it is posted. I hope that the participants will find it as informative as some of the staff have already expressed that it has been to them.

This rotation with the Ryan Center was a great way to finish my dietetic internship. I was able to apply skills that I developed at my first three rotation sites to tasks that I was assigned with here. I hope to continue to improve throughout my career applying what I’ve learned each step of the way to each position I hold. The time I’ve spent in the wide range of rotation sites I’ve been fortunate enough to work in has prepared me to face whatever challenges are ahead of me.
