This facility operates in three locations here in the South Bronx. The two main locations that I will be working at are the South Bronx Health Center and the Center for Child Health and Resiliency. There are many different programs offered here that I will be a part of which should provide a lot of valuable learning experiences with various populations.

Every Friday I will be responsible for creating a salad of the week based on a current topic from the Academy’s website. Going around the office to sign people up and collect money is a good exercise in getting to know the staff. It is encouraging that the majority of the staff signed up for the salad this week. The HFG program is very similar to the type of program I conducted at a Boy’s and Girl’s Club during my undergraduate studies. Unfortunately for me, this was the last meeting for this session so I did not get to observe a normal lesson is run. I will be able to observe another session next week however.

The salad was well received by the staff. It was challenging to prepare all of the salads on time especially when there were two locations involved but next week I will get an earlier start and I’ll also work across the start at the other office where there is more space available in the break room. Sandra clearly explained to me that she expects me to be available within the health center at all times and that I should let her know if I am going out to do any food shopping. She explained the concept of harm reduction to me which is very important in a community setting. In order to not discourage your clients she explained that you have to assess their socioeconomic status and physical ability to provide and prepare food. It could cause more harm than good to suggest that a client change their diet in ways that are not attainable to them causing them to lose hope in the process and fail to improve at all and possibly even experience negative results such a feeling of helplessness.

I had the opportunity to observe the other dietitian here, Diana, conduct an assessment with a patient. We also went over the charting process.

Diana mostly sees pediatric patients at the health center but also see HIV patients once a week. The patient we saw together was a child underweight for their age. A recommendation was given to provide an extra snack between breakfast and lunch and to monitor the intake of the child. A follow up appointment was made for two weeks when the child’s weight will be reevaluated.

Participated in Baby group. This week’s meeting was the birthday party for the children who are all turning one. I prepared omelets for the children and their parents. I came up with an idea for this week’s salad. HFG with a younger group of children, around five to seven years old. In this meeting we made hummus, discussed fruit and vegetable intake, and had time in the gym.

Both of the groups today that I participated in were very well organized. It is apparent that the staff here at the center have been conducting these groups for a while. It was a good first experience to the Baby group cooking the omelets and describing the ingredients that were used. The HFG session was a lot of fun and the children, about 12 of them, were all very interested in our lesson and the food activity.

The Shop Healthy NYC initiative through the NYC Department of Health and Mental Hygiene has been working with SBHC in two nearby grocery stores: Western Beef and Real. There is a desire to expand into a nearby corner store that I will be visiting soon with Marlene, the other intern here, to gauge their interest in participating in the initiative. The flyers for the Super Star salad are a great way to provide nutrition education to the entire staff each week while promoting the salad. They are posted in the break rooms and I will use them when asking people to sign up for the salad.

The CAB meeting was a great learning experience in working with community members to improve service provided by the health center. I had the opportunity to speak with a member of the community about their concerns over the lack of quality after-school programs. I informed them about the HFG meetings that are offered by the health center.

The Super Star salad will be a challenge each week to create salads that appeal to the majority of the employees at the health center. Almost 30 people signed up for this week’s salad and I took note of several preferences and food allergies that I accommodated when preparing the salads. I am looking forward to working more on the project assigned to me. It will be a good way for me to become even more familiar with the cultural practices of the heath center’s clients in addition to regular day-to-day contact with them.

The WAW is a presentation that is conducted in the waiting room of the health center with the patients. The intent is to provide a quick, interactive lesson to the waiting patients. Our ideas for lessons include sugar-sweetened beverages and making MyPlate more relatable to the population of clients at the health center.

Since this morning’s group was cancelled the other intern and I decided that cleaning the nutrition area in the basement would be a good use of our time. We discarded the products that were outdated and organized what was left. This allows for us to better utilize the items that we have for upcoming groups so that there will be less waste the next time this area is cleaned up.

Today I two patients with Diana and charted on them myself. The patients were both adolescents with overweight diagnoses. I repurposed unused food from Baby Group for this evening’s MLP meeting.

The patients that I saw today were diagnosed with the same condition but the counseling strategies used with them were completely different. One of the patients had a parent with them and the focus was on how the family as a whole can help improve the food environment in their household. The other patient was at the appointment alone and the strategy used was find out what negative food habit they most wanted to stop. It was determined that the patient would no longer buy snacks late at night and instead save the money for a new iPhone.

I spent time this morning working on my project and have found the national dishes of many of the Hispanic countries. Marlene and I went to the convenience store, D’Sosa Gourmet Deli, to talk with the owner about the Shop Healthy initiative.

The Shop Healthy site visit went very well. The owner was very responsive to the ideas we pitched to her and allowed us to take a quick survey of the store to determine which areas they may need help improving on. I was not able to talk with the owner very much because she mostly spoke Spanish. Marlene was able to communicate with her however and I tried to pick up as much as I could.

Marlene and I have decided to use our MyPlate idea for the WAW. We are naming the session, “Making MyPlate “OurPlate.” We have agreed on a budget for the session that will include the cost of the supplies (paper plates, pencils, food models) that we’ll need. We have also divided up and assigned portions of the presentation and activity in order to know who is conducting each part and how much time we’ll need.

The Baby group this morning was very surprised to see how easy it was to prepare a frittata. Everyone in the group enjoyed the snack and most said that they would try it at home. I handed out copies of the recipes for them to use and provided some encouragement for them to try it.

Jenny, who has been involved with the Shop Healthy initiative for a while, wanted to know the outcome of our site visit to D’Sosa. The email I sent her will provide the information needed to make a decision on what the next steps should be. In my email I suggested a list of possible next steps and will discuss them with Jenny when she has had time to review them. Leading the activity during this morning’s Pregnancy group was a lot of fun. The patients were involved in the activity answering questions I tested them with and asking questions of their own. The interaction between myself and the patients makes for a much more successful session.

A pattern has emerged in my project now that I’ve been working on organizing each country’s staples in the major food groups. The Central American countries have a heavier dependence on starches such as rice and beans and the South American countries rely more heavily on animal products such as beef, fish, and milk. The information Diana was able to give me on past snacks for the HIV group that were well received was very helpful in planning this week’s meeting.

Today was focused completely on food shopping and preparation. I enjoyed the challenge of shopping for, preparing, and developing different recipes for different groups each with their own nutritional needs.

Food prep for Pregnancy group. Led calorie guessing activity and recap on health eating choices at fast food restaurants. Conducted WAW. Several parents listened to our lesson and their children participated in putting together a plate that matched MyPlate while incorporating familiar foods.

Both presentations I led today went well. The WAW was definitely the more challenging of the two. The Pregnancy group was a planned session of patients expecting to receive a nutrition lesson during their visit. The patients in the waiting room were unaware of the lesson and several refused to participate. Of the other patients that did participate the majority of them seemed to enjoy the activity and expressed that they learned some useful information on how to make their plates at home healthier.

There was an overwhelming consensus today from the health center staff that the salad dressing should no longer be put on the salad when it is prepared. Instead I am now putting the dressing on the side in a cup with a lid for the staff to portion out themselves when they are ready to eat their lunch. This is a small change that was easy to make and actually makes for less work on my part while improving the quality of the product being produced.

Some changes that I have made to my project to improve it include removing Spain as it is not truly a Hispanic country for the purposes of the health center’s population. Another change was to add more than one national dish for some of the countries that have several identified with them. This also improves the project by providing a more comprehensive picture of this population’s food preferences.

Some feedback provided to me by Diana included working on my introduction and getting a better start to the session. I need to work on introducing myself and determining the reason of the patients visit. The patient I saw with Sandra was a pregnant woman who is concerned about her rate of weight gain during her pregnancy. She has not been gaining an adequate amount of weight and attributes it to nausea and vomiting. Recommendations to use crackers to help with nausea and the consumption of multiple small meals throughout the day were given.

The patients in tomorrow’s Pregnancy group are self-described as picky eaters so it has been challenging to come up with healthy recipe ideas that are also appealing to the group. The recipe I decided on is a Savory Sweet Potato Soup.

For today’s Pregnancy group I was given the opportunity to lead the session organizing the snack and the nutrition lesson. In order to complete the snack on time before presenting the nutrition lesson, which were scheduled back to back, I needed to enlist the help of the AmeriCorps fellows and the other intern , Marlene. I directed each of them to help with the food preparation and helped to serve the soup before presenting the nutrition lesson.

The patient that I saw today described their dietary recall and I was able to educate them on the benefits to including protein with their carbohydrate choices to help regulate blood glucose levels. The patient also expressed a desire to control their diabetes through diet due to an aversion to taking medication. I provided the patient with recommendations on following the MyPlate model while slightly decreasing the carbohydrate serving size and slightly increasing the protein serving size.

This morning’s Baby group was very small with only three patients present. When the time came for me to conduct the lesson on growth charts one patient was having a procedure done with the nurse and another patient was receiving assistance with breastfeeding issues she is having which left one patient for me to do the lesson with. I wanted to wait until the other patients were able to join in but there was not enough time remaining in the session. I appreciated the fact that I could have a one-on-one session with a patient but it would have been good to provide the education to the other patients as well.

The group Pregnancy group today showed very little interest in the nutrition lesson about sugar in various beverages. However, once I started the demonstration pouring the actual amounts of sugar in each drink into clear plastic cups the group showed interest. This demonstration was very effective in eliciting a response from the group and surprised the patients very much. The majority of them expressed a newfound concern over their sugar intake.

The handouts I created today for my presentation next week are intended as a guide for the providers to use with their patients following the presentation. They are a summary of the staples foods and national dishes of each country and will give the provider’s a starting point to talk about nutrition with their patients if they choose to do so.

This was my last Super Star salad today that I will make here at SBHC. I have grown to really appreciate this idea. It has enabled me to share many short nutrition lessons with the staff through the posted flyers and conversations while collecting money. This, in addition to increasing vegetable and fruit consumption of the staff, is a great benefit to the health center overall and its relationship with the dietitians and their work with nutrition.

In order to entice the provider’s to attend my presentation we are providing food. The food that will be served is inspired by the project. We are ordering the national dishes from two of the countries: pupusas (El Salvador) and empanadas (Chile). Another reason to provide the food is to help form a memorable experience which will hopefully relate into the application of the information learned.

Food prep and room preparation for presentation. There were nine providers present for the actual presentation and five more came by individually afterwards. I was able to discuss the main points of the presentation with them and gave them each a copy of the handout.

My presentation went very well today. Many of those present stated that they had learned a lot of new information on the traditional dietary practices of their patient population. The handout was a success with several providers commenting positively on it and its potential usefulness. This project that started out simply as a way for me to increase my familiarity with Hispanic culture and food grew into much more and with this presentation I was able to share what I had learned with others who will hopefully now be better suited to address the nutritional needs of their patients.

In the event that Oni is unable to make it here tomorrow as scheduled for training I spend the day today working on transitional instructions. The work of the interns here at SBHC is varied and extensive. The number of groups the interns are involved with and the requirements for each are a lot to figure out at first. It will be very helpful for new interns to have instructions on the different groups, what is expected from them, and who the contact person is for each one.

Food prep for Pregnancy group. Also, led nutrition lesson for the group. Training with Oni.

 

Comments / Thoughts / What You Learned / Challenges:

For my final nutrition lesson I was given a very short period of time to present. The group was running behind schedule so I had to adjust at the last minute to cover the most important messages in the time available. I feel that I was able to successfully convey the general idea behind the lesson and that the patients understood the key points. It was not a good example for Oni to observe but it was a good lesson on the work environment of community nutrition where anything can change at any time and the need to be prepared for whatever comes your way. I informed Oni of the different groups she will be involved with and what the expectations would be of her. It is obvious that Oni will be an asset to SBHC.
