Overview of Clinical Rotation
My final rotation was at Lincoln Hospital and Mental Health Center in the Bronx. In my time here, I spent time with six dietitians in various capacities. I rotated through surgical, pediatric, maternity, mental health, internal medicine, and intensive care units, observing and treating a variety of patients. I became familiar with the EMR charting software Quadramed, which does not encompass a particular nutrition charting format, but did include elements of ADIME chart notes. Each dietitian utilized different methods of assessment, intervention, and education, and I learned a great deal from each preceptor. I found this rotation to be challenging intellectually as well as emotionally, and while I now feel competent in a clinical setting, I doubt my heart could work in such an environment for much longer. This rotation was a valuable experience, but I won’t be returning to clinical work anytime soon.

This WIC Works design is so confusing! Sometimes you take the pre-test and sometimes you don’t sometimes it has you take the after-test and sometimes you have to do it yourself, sometimes there are questions within the modules but do those count for a test? I have no idea! It took me a little while to realize that I didn’t have to listen to the lady talk, but that all the words are on the slide and I can just read them myself. I didn’t realize these trainings would take so long – even if it’s simple material, there’s so much you have to go through if you actually want to retain all the information.

I feel like I’m learning so much from WIC about raising a baby, which is just good life advice! But I also feel like some of it might be outdated. The emphasis on breastfeeding is made very clear, but there was one section on positioning a baby for sleep. I’ve learned in some anthropology courses that the evidence supporting SIDS as caused by putting a baby to sleep on its stomach is not very sound. But that’s minor thing.

I’m not scared of needles, but my body does not handle that type of pain very well. But at least I had time to finish the WIC training! That took way too long.

Today I observed the diet office taking and carrying out all orders for patients. It’s a very interesting process – one that needs to be updated! There are 5 people whose sole jobs it is to manually check and change every order – shouldn’t this be automated? But everyone is really nice there.

So much work is on the computer. And most of the interactions with patients is so minimal and the education is not helpful at all. Some of the patients just tell you what you want to hear, and you can tell, but you can’t do anything about it. It’s weird. I don’t know how I’m going to function here with limited real human being interactions with patients.

Today I got my first patient – it’s very simple to assess/treat patients that are NPO. Haha. But I think Mariana is also realizing I am useful – to lighten her workload, not add to it. So she’s giving me progress notes to chart and other tasks to do, which is good – I feel useful and not so bored and sleepy.

Today felt like starting all over again – ICU cases are much, much different, and Brooke has a very different approaches and uses different tools than everyone else. It’s hard because I feel like this is the way it’s supposed to be – there’s a right answer for everything based on evidence guidelines. But that’s not always how it works on other floors. It’s just frustrating.

Today I realized how arbitrary recommendations are. Five different dietitians can give five different ones and not one of them might be right. I guess that’s what clinical judgment is for, but how can you exercise clinical judgment if someone is constantly correcting yours to look like their own?

Today I covered MICU by myself, which was super easy since most of the patients are noncommunicative or NPO and really, should we be trying to talk to patients that are in intensive care? Only if we need to for safety/medical reasons, like tolerance of diet or being able to swallow. Which, most of the time, you can find out from the nurse.
