Overview of Clinical Rotation

My final rotation was at Lincoln Hospital and Mental Health Center in the Bronx. In my time here, I spent time with six dietitians in various capacities. I rotated through surgical, pediatric, maternity, mental health, internal medicine, and intensive care units, observing and treating a variety of patients. I became familiar with the EMR charting software Quadramed, which does not encompass a particular nutrition charting format, but did include elements of ADIME chart notes. Each dietitian utilized different methods of assessment, intervention, and education, and I learned a great deal from each preceptor. I found this rotation to be challenging intellectually as well as emotionally, and while I now feel competent in a clinical setting, I doubt my heart could work in such an environment for much longer. This rotation was a valuable experience, but I won’t be returning to clinical work anytime soon.

Location: Home

Hours: 7

Activities/Assignments:

Uploaded journals to e-portfolio and formatted for clarity

Began WIC works training

Formatted last newsletters for Food Bank

Comments/Thoughts/Lessons/Challenges:

This WIC Works design is so confusing! Sometimes you take the pre-test and sometimes you don’t sometimes it has you take the after-test and sometimes you have to do it yourself, sometimes there are questions within the modules but do those count for a test? I have no idea! It took me a little while to realize that I didn’t have to listen to the lady talk, but that all the words are on the slide and I can just read them myself. I didn’t realize these trainings would take so long – even if it’s simple material, there’s so much you have to go through if you actually want to retain all the information.

Worked on WIC modules

Uploaded Food Bank projects to and organized in e-Portfolio

Comments/Thoughts/Lessons/Challenges:

I could not bring myself to work today, I’m sick and exhausted and just want to lay in bed forever. Organizing stuff on this website is really more complicated and time-consuming than it has to be.

Learning Objectives Met:

CRD 1.2, 2.1, 2.4, 2.12, 2.13, 3.2, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/12

Uploaded and formatted more journals in e-portfolio

Worked on WIC modules

Comments/Thoughts/Lessons/Challenges:

WIC modules are never-ending!!! I’m not even halfway done!

Learning Objectives Met:

CRD 1.1, 1.2, 2.1, 2.4, 2.5 2.12, 2.13, 3.2, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/13

Rotation: Lincoln Hospital

Location: Home

Hours: 2

Activities/Assignments:

WIC modules

Worked on e-portfolio gallery

Comments/Thoughts/Lessons/Challenges:

So, getting sick has made me infinitely less productive. Taking some time to recuperate, need lots of sleep (at this point, we should just call it hibernation).

Learning Objectives Met:

CRD 1.2, 2.1, 2.4, 2.12, 2.13, 3.2, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/17

Rotation: Lincoln Hospital

Location: Home

Hours: 7

Activities/Assignments:

WIC Modules

Orientation homework for clinical rotation

Comments/Thoughts/Lessons/Challenges:

I feel like I’m learning so much from WIC about raising a baby, which is just good life advice! But I also feel like some of it might be outdated. The emphasis on breastfeeding is made very clear, but there was one section on positioning a baby for sleep. I’ve learned in some anthropology courses that the evidence supporting SIDS as caused by putting a baby to sleep on its stomach is not very sound. But that’s minor thing.

Learning Objectives Met:

CRD 1.2, 1.5, 2.1, 2.4, 2.13, 3.2

--------------------------------------------------------------------------------------------------------

Date: 3/18

Rotation: Lincoln Hospital

Location: Hospital/Home

Hours: 9

Activities/Assignments:

WIC modules

Medical Clearance and other administrative stuff

Uploading to and organizing e-Portfolio

Comments/Thoughts/Lessons/Challenges:

So I couldn’t actually start working at the hospital today because I had to get two vaccines, on top of getting titers and ppds and everything else. I’m not scared of needles, but my body does not handle that type of pain very well. But at least I had time to finish the WIC training! That took way too long.

Learning Objectives Met:

CRD 2.4, 2.12, 2.13, 3.2, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/19

Rotation: Lincoln Hospital

Location: Hospital

Hours: 7

Activities/Assignments:

Received flu shot

Introduced to kitchen and dietitian staff

Tour of kitchen

Read nutrition policy and procedure manual

Filled out paperwork

Introduced to intranet resources

Comments/Thoughts/Lessons/Challenges:

Today was my first official day at the hospital. The staff is all really nice, bu I’m feeling very overwhelmed by the amount of systems and things I have to learn. I suppose it will get easier once I start working with the dietitians.

Learning Objectives Met:

CRD 2.1, 2.5, 2.9, 2.10, 2.11, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/20

Rotation: Lincoln Hospital

Location: Hospital

Hours: 7

Activities/Assignments:

Got a new lab coat!

Turned in paperwork to volunteer office

Observed diet office work – carrying out new diet orders/change requests

Introduction to computer/Quadramed use

Saw the library

Comments/Thoughts/Lessons/Challenges:

Today I observed the diet office taking and carrying  out all orders for patients. It’s a very interesting process – one that needs to be updated! There are 5 people whose sole jobs it is to manually check and change every order – shouldn’t this be automated? But everyone is really nice there.

Learning Objectives Met:

CRD 2.1, 2.5, 2.9, 2.10, 2.11, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/21

Rotation: Lincoln Hospital

Location: Hospital

Hours: 8

Activities/Assignments:

Observed dietitian rounds and charting

Comments/Thoughts/Lessons/Challenges:

So much work is on the computer. And most of the interactions with patients is so minimal and the education is not helpful at all. Some of the patients just tell you what you want to hear, and you can tell, but you can’t do anything about it. It’s weird. I don’t know how I’m going to function here with limited real human being interactions with patients.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.9, 2.10, 2.11, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/23

Rotation: Lincoln Hospital

Location: Home

Hours: 4.5

Activities/Assignments:

Wrote Service Improvement Project

Uploaded Service Improvement Project to e-Portfolio

Comments/Thoughts/Lessons/Challenges:

I thought writing the Service Improvement project was going to be way more extensive than it was. Now I’m done with it, and all I really have left is modules and chart reviews! Pretty clean schedule from now on.

Learning Objectives Met:

CRD 1.3 Justify programs, products, services and care using appropriate evidence or data

CRD 4.6 Analyze quality, financial or productivity data and develop a plan for intervention

CRD 4.7 Propose and use procedures as appropriate to the practice setting to reduce waste and protect the environment

CRD 1.1, 1.2, 1.4, 1.5, 2.2, 2.8, 2.9, 2.10, 2.13, 3.2, 3.3, 3.4, 4.4, 4.5, 4.8, 4.9, 4.10

--------------------------------------------------------------------------------------------------------

Date: 3/24

Rotation: Lincoln Hospital

Location: Hospital/Home

Hours: 9.5

Activities/Assignments:

Observed chart review, patient assessment/re-assessment/consult, chart documentation

Explored Quadramed software and practiced finding relevant patient data

Worked on Foodservice Module

Comments/Thoughts/Lessons/Challenges:

Bureaucracy. At least I get a workout running around the hospital getting forms signed and access granted.

Learning Objectives Met:

CRD 2.1, 2.5, 2.9, 2.10, 2.11, 4.5

--------------------------------------------------------------------------------------------------------

Date: 3/25

Rotation: Lincoln Hospital

Location: Hospital/Home

Hours: 10.5

Activities/Assignments:

Assessed patient, documented assessment; documented re-assessment

Provided care plan for patient (intervention, monitoring)

Surveyed patient satisfaction with meals

Observed assessment/documentation

Worked on foodservice module

Comments/Thoughts/Lessons/Challenges:

Today I got my first patient – it’s very simple to assess/treat patients that are NPO. Haha. But I think Mariana is also realizing I am useful – to lighten her workload, not add to it. So she’s giving me progress notes to chart and other tasks to do, which is good – I feel useful and not so bored and sleepy.

Learning Objectives Met:

CRD 3.1b Diagnose nutrition problems and create problems, etiology, signs and symptoms (PES) statements.

CRD 3.1d Monitor and evaluate problems, etiologies, signs, symptoms and the impact of interventions on the nutrition diagnosis.

CRD 1.1, 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1(a,c,e), 3.2, 4.4

--------------------------------------------------------------------------------------------------------

Date: 3/26

Rotation: Lincoln Hospital

Location: Hospital/Silberman

Hours: 9

Activities/Assignments:

Assessed patients, documented assessments, provided care plan

Attended “huddle”

Worked on GI module, worked on chart review

Comments/Thoughts/Lessons/Challenges:

There’s like this management thing Sodexo does where all the kitchen staff (including the dietitians) meet and discuss a work topic – today was safety from being taken advantage of…very interesting.

Learning Objectives Met:

CRD 1.2, 2.1, 2.4, 2.5, 2.7, 2.8, 2.9, 2.10, 2.11, 3.1a-e, 3.2

--------------------------------------------------------------------------------------------------------

Date: 3/27

Rotation: Lincoln Hospital

Location: Hospital

Hours: 8

Activities/Assignments:

Re-assessed patients and documented care plan

Met the union contact

Surveyed patients for meal satisfaction

Observed assessment/documentation

Comments/Thoughts/Lessons/Challenges:

I hate this. I hate being cold and straightforward with patients to get the answers you need. I always feel like patients must think we can’t wait to run out the door on them. Awful. This is why I can’t work in a hospital. Maybe a nursing home would be okay.

Learning Objectives Met:

CRD 1.1, 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

--------------------------------------------------------------------------------------------------------

Date: 3/28

Rotation: Lincoln Hospital

Location: Hospital

Hours: 7

Activities/Assignments:

Assessed, re-assessed, observed

Pt satisfaction surveys

Comments/Thoughts/Lessons/Challenges:

Today was a good day. Mariana let me talk to patients on my own today, which didn’t go horribly, so I feel good about that.

Learning Objectives Met:

CRD 1.1, 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

--------------------------------------------------------------------------------------------------------

Date: 4/7

Rotation: Lincoln Hospital

Location: Hospital/Home

Hours: 9

Activities/Assignments:

Observed assessment and documentation of patients

Finished foodservice and GI modules

Comments/Thoughts/Lessons/Challenges:

Today was my first day back from break, it was super boring and I don’t see it getting any better. I’m just not cut out for this type of job. I wish I was back at Renfrew, which makes me feel crazy.

Learning Objectives Met:

CRD 2.1, 2.5, 2.9, 2.10, 2.11, 4.5

--------------------------------------------------------------------------------------------------------

Rotation: Lincoln Hospital

Location: Hospital/Silberman

Hours: 9

Activities/Assignments:

Assessed, re-assessed, documented

Pt satisfaction surveys

CVD module

Comments/Thoughts/Lessons/Challenges:

Today was a little better, let’s call it a recovery from yesterday. There was this sad sad couple that the husband had a massive stroke and the wife was talking nonstop about his improvement and eating and sounded so hopeful about his prognosis and it broke my heart a thousand times.

Learning Objectives Met:

CRD 1.1, 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Rotation: Lincoln Hospital

Location: Hospital

Hours: 8.5

Activities/Assignments:

Assessed, re-assessed, documented

Attended interdisciplinary nutrition committee

Comments/Thoughts/Lessons/Challenges:

Today was so crazy with meetings and patient problems. Anita’s managerial duties are so interesting – I feel like she’s trying to measure productivity but I could already tell you just by observing the last couple of weeks who does more work and who does just as much as they have to.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

--------------------------------------------------------------------------------------------------------

Date: 4/12

Rotation: Lincoln Hospital

Location: Home

Hours: 6

Activities/Assignments:

Worked on chart reviews

Worked on CVD/diabetes modules

Comments/Thoughts/Lessons/Challenges:

I have 5 chart reviews done already! I’m glad that I’m not struggling to find these and that I can hopefully finish them early so I can just focus on learning and not swiping patients with diseases I need. And, now that I have internet access at work, I can work on the modules during lunch, so I don’t have to put in as many extra hours outside of the 40-hour weeks I’m already working because I’m going to be done with my hours with two weeks left at this rotation. I should’ve asked for Fridays off.

Learning Objectives Met:

CRD 1.2, 1.4, 3.1a-e

--------------------------------------------------------------------------------------------------------

Date: 4/13

Rotation: Lincoln Hospital

Activities/Assignments:

Assessed, Re-assessed, documented

Finished DM, renal modules

Comments/Thoughts/Lessons/Challenges:

The most infuriating thing – being interrupted in the middle of an assessment by a pretentious doctor. I told that bastard to wait.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Date: 4/15

Rotation: Lincoln Hospital

Location: Hospital

Hours: 8

Activities/Assignments:

Assessed, re-assessed, and documented

Worked on oncology module

Comments/Thoughts/Lessons/Challenges:

So, I’m becoming frustrated here, because I feel like the goal is to do as little work as possible, but that means I get really bored very quickly. I spent almost two hours today working on modules because I don’t have enough work to do. Maybe I should ask for more. But then I also feel like the dietitians don’t want me to take their work. I don’t know, it’s complicated. I’m running out of things to do.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Comments/Thoughts/Lessons/Challenges:

Done with this preceptor, on to another! Today I did 5 patients and managed to leave early to catch a bus. I’m feeling a lot better about my abilities and competency at this job, but I’m also still so done with this rotation. I never, ever want to work in a hospital setting like this. Now I just need to get through the next SIX weeks. Six has never felt like such a huge number before.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Today was my first day with Debra, and it was a really good learning experience. I feel like I’m going to learn a lot with her. I also didn’t feel like the day dragged by. I also kind of love the psych unit.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 2.12, 3.1a-e, 3.2, 4.4, 4.5

Hours: 9

Activities/Assignments:

Comments/Thoughts/Lessons/Challenges:

I am so exhausted. I might need to take a day off, I look like Rudolph and my head feels like it’s in a pressure cooker.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Comments/Thoughts/Lessons/Challenges:

Today went really well. I am feeling more comfortable with patients, although it’s difficult to feel relaxed when someone is standing next to you just watching. I’m also feeling better today, got lots of sleep last night. Hopefully I can survive until next week.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Hours: 9

Activities/Assignments:

Assessed, re-assessed and documented

Comments/Thoughts/Lessons/Challenges:

I cannot seem to get this preceptor to let me leave on time! It’s really frustrating since I even finished my work early. I just ended up sitting there for 2 hours doing module stuff waiting for her to check my notes.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Date: 5/1

Rotation: Lincoln Hospital

Location: Hospital

Hours: 9

Activities/Assignments:

Assessed, re-assessed and documented

Comments/Thoughts/Lessons/Challenges:

I am not getting better. I don’t know what to do to kick this cold! I feel weak and useless. But today I wrote 6 notes, which is the most I’ve ever written. Tomorrow there will only be 2 dietitians on staff, so I’ll get to take on a lot of work. I wish I wasn’t feeling crappy, but I feel ready to be on my own.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Rotation: Lincoln Hospital

Location: Hospital

Hours: 7.5

Activities/Assignments:

Assessed, re-assessed, documented

Comments/Thoughts/Lessons/Challenges:

Today there were only two dietitians at work so I got to be a half-real dietitian and did 6 patients all by myself! Well, with Mariana talking to the Spanish-speaking ones. I survived and now I am going to hibernate all weekend.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Date: 5/5

Rotation: Lincoln Hospital

Location: Hospital

Hours: 8.5

Activities/Assignments:

Introduced to SICU staff and procedures

Observed assessment and documentation of SICU patients

Comments/Thoughts/Lessons/Challenges:

Today felt like starting all over again – ICU cases are much, much different, and Brooke has a very different approaches and uses different tools than everyone else. It’s hard because I feel like this is the way it’s supposed to be – there’s a right answer for everything based on evidence guidelines. But that’s not always how it works on other floors. It’s just frustrating.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.9, 2.10, 2.11, 4.5

--------------------------------------------------------------------------------------------------------

Date: 5/6

Activities/Assignments:

Assessed and documented on SICU/maternity patients

Reviewed evidence base and protocols for nutrition support

Comments/Thoughts/Lessons/Challenges:

Today Brooke took the time to go over the enteral/parenteral feeding practice guidelines with me. This is the first time I feel like I’m learning like I used to, which makes me feel a thousand times better than the past two months have.

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

I hate this place. I got here at 6:30 and no one was here. Because nobody sent me the revised schedule that said my preceptor for today would be out. FML.

Comments/Thoughts/Lessons/Challenges:

Today I realized how arbitrary recommendations are. Five different dietitians can give five different ones and not one of them might be right. I guess that’s what clinical judgment is for, but how can you exercise clinical judgment if someone is constantly correcting yours to look like their own?

Learning Objectives Met:

CRD 1.2, 2.1, 2.5, 2.6, 2.9, 2.10, 2.11, 3.1a-e, 3.2, 4.4

Comments/Thoughts/Lessons/Challenges:

Today I covered MICU by myself, which was super easy since most of the patients are noncommunicative or NPO and really, should we be trying to talk to patients that are in intensive care? Only if we need to for safety/medical reasons, like tolerance of diet or being able to swallow. Which, most of the time, you can find out from the nurse.
